Night call.

The night is cold and silent. You're on patrol with your partner, Tim. The clock reads 2:15 a.m. The radio clackle; "Unit 25, road traffic accident, single vehicle and driver unconscious." Tim starts driving, you take a deep breath and prepare yourself mentally. Trauma, bleeding, emergency, "we're here". Grab the full medical kit immediately, or grab the essentials to move faster.

A car is crashed into a pole, the engine is smoking and a man sits inside. You notice that he is motionless, so you have to be quick! As you try to open the door, you realize that you have another problem... The door is jammed. You try to open the door by yourself or you call the firefighters?

The essentials.

You've decided to grab the essentials and as you wanted, you arrived fast to the car. You see a man motionless, sitting in the car, but you can't open the door because it is jammed. As you reached to your radio to call some help to open it, you realize that you forgot your radio in the truck... You go back and grab it, but when you came back, the man was dead. Start again!

You open the door by yourself.

While trying to open the door by yourself, you hurt yourself badly, and you have to leave the call. You could not save the man. Start over.

You called the firefighters.

The firefighters come and manages to create an opening. "Sir? Can you hear me?" No response. You check quickly; weak breathing, fast pulse and blood is running down from his forehead. Either you decide to focus on his breathing or you try to open the door a little bit more to have better access to the man.

You've decided to focus on his breathing. He's breathing really poorly, he is unconscious, and possible head trauma. Tim looks at you; " do we extract him or stabilize him here?" The car engine starts crackling faintly. Extract him right now or stabilize him inside the vehicle.

Extraction.

You decide to extract him. You and Tim manage to pull the man out. He groans slightly, he's alive, but he is still critical.

You choose to stabilize him in the car. You hold his head steady and adjust his breathing, but time passes. The engine keeps crackling and the situation becomes dangerous. Eventually you have no choice, but to extract him.

The man is now on the ground. You apply oxygen, but the bleeding is getting under control! He does not regain consciousness. Now you have a big decision to take. Load him into the ambulance immediately or continue assessing for a few more minutes...

Inside the ambulance, the atmosphere is tense. Tim is driving really fast with the alarm and the flashes on. "5 minutes out". The monitor beeps, unstable rhythm, and weak breathing. You prioritize airway management.

Sadly the patient is deterioring fast... You try to bring him in the truck but it is too late.. He's dead. Start again!

You secure the airway! Air flows better and the heart rhythm gradually stabilizes. At the hospital, the team takes over. A few minutes later: "He is stable. You made the right call." Tim gives you a tired smile, you start breathing normally again, one more life saved. Different choices, different outcomes. Because in this job, every decision matters! Start again.
