consent form
A document signed to show agreement to something e.g. a form signed by a patient prior to a medical procedure to confirm that he or she agrees to the procedure and is aware of any risks that might be involved.
Example: Parents needed to sign a consent form to give permission for their child to go on a trip to Alice Springs.
